Why does primary care matter for seniors?
Your primary care provider (PCP) is your main doctor for checkups, screenings and non-emergency care, focusing on your whole health. As you age or develop chronic conditions, your value-based care PCP coordinates with specialists to keep your healthcare personal and proactive. At Humana, we believe this relationship is key to positive health outcomes.
I already have a specialist. Why should I get a PCP too?
It’s great to have a specialist you trust, but their focus is narrow. A PCP takes a holistic view of your whole health, diagnosing, treating and preventing a wide range of conditions. The best part? Your PCP and specialists work together to support your best health.
I’ve been going to the same doctor for years. Why should I consider a new doctor as I get older?
Just as kids see pediatricians, older adults benefit from doctors like geriatricians, who specialize in care for aging bodies. They focus on your overall well-being, helping manage chronic conditions, medications and health goals.
What is value-based care and how could it help me?
Value-based care focuses on quality over quantity, paying doctors for patient outcomes rather than the number of visits or procedures to achieve:1
  1. Better individual care
  2. Better population health
  3. Lower costs
Humana’s Value-based Care Report shows members in these arrangements had the following compared to those in other plans:2
  • Better medication adherence
  • More screenings
  • Fewer hospital stays
  • Lower ER visits
With value-based care, your well-being comes first through a proactive, team-based approach that could lead to better health at a lower cost.
How can I become a patient with a value-based care provider?
Becoming a Humana Medicare Advantage member is a simple way to gain access to value-based care providers in your area.
  1. Reach out to a licensed Humana Medicare sales agent. There’s no cost to you to work with an agent and no obligation to enroll in a plan.
  2. They will help you find the right plan for you and your needs and can also help you find value-based care providers who are conveniently located to you.
I’m already on Original Medicare. Why should I consider a Medicare Advantage plan?
Original Medicare covers hospital and preventive care but has gaps in dental, vision, hearing, and drug coverage—and no cap on out-of-pocket costs. A Humana Medicare Advantage plan can fill those gaps, offering potential extra benefits like prescription drugs, dental, vision, hearing, fitness, and/or wellness support, plus access to a growing value-based care network. Benefits vary by plan and location. Many Medicare Advantage plans offer low or $0 premiums to stay competitive. You’ll still pay your Part B premium. Private insurers also set your deductible, coinsurance and copays. Reach out to a licensed Medicare sales agent to find the right Medicare Advantage plan for you. You can enroll during the Annual Election Period (AEP) which is October 15–December 7 annually or during a Special Election Period if you qualify for one.
